World Patient Safety Day: Safe medication, health accompanies!

September 17, 2022, is the fourth World Patient Safety Day, with the theme "Medication Safety" and the slogan "Avoid Medication Harm."

September 17, 2022, is the fourth World Patient Safety Day, with the theme "Medication Safety" and the slogan "Avoid Medication Harm."Unsafe medication use and medication errors are major causes of harm and preventable harm in healthcare systems around the world.

The main goals of this event include: raising awareness in society about the harm caused by medication errors, popularizing medication safety knowledge, encouraging patients and their families to participate in self-management of medication safety, and taking effective measures to improve the level of patient medication safety. It is hoped that achieving the goals of the event will enhance the medication safety management level of medical institutions, increase public awareness of medication safety, popularize safe medication knowledge, prevent medication errors, and reduce medication-related harm.

As a special type of commodity, drugs are subject to strict regulation at all stages of research, production, sales, and use. However, in our daily use of medications, medication harm can still occur due to issues such as duplicate dosing, interactions, and dosage.

To minimize harm caused by improper medication use, we recommend a medication safety assessment tool - the "Five Key Moments of Medication Safety," which reminds patients or healthcare personnel of five key moments to remember when using medications. At different moments, specific questions should be raised and answers sought to reduce medication-related risks. These five moments are:Recognizing medications, taking medications, adding medications, checking medications, stopping medications.

 

1. Recognizing Medications

① What is the name of this medication, and what is its function?

② What are the risks of taking this medication, and what side effects might occur?

③ Are there other treatment options for my condition?

④ Have I informed the doctor of my allergy history and other health conditions?

⑤ How should I store this medication?

2. Taking Medications

① When should I take the medication, and what is the dosage for each time?

② How should I take this medication?

③ Do meals and drinks affect the medication I am taking?

④ What should I do if I miss a dose?

⑤ What should I do if I experience side effects?

3. Adding Medications

① Do I really need to add other medications?

② Have I informed the doctor about the medications I am currently taking?

③ Will the medications I am currently taking interact with each other?

④ What should I do if I suspect there is an interaction?

⑤ Can I manage multiple medications correctly?

4. Checking Medications

① Do I keep a list of my medications?

② How long have I been taking each medication?

③ Am I taking any medications that I no longer need?

④ Are my medications checked regularly?

⑤ How often should my medications be checked?

5. Stopping Medications

① When should I stop taking the medication?

② Are there any medications that should not be stopped suddenly?

③ What should I do if I run out of medication?

④ If I stop taking medication due to adverse reactions, where should I report it?

⑤ How should I dispose of excess or expired medications?

 

Safety has always been an abstract concept, "invisible and intangible." In the minds of many, hospitals are a "safe" place, while for hospital managers, the safety of the hospital is based on a crisis awareness that is implemented in every detail.
